If you prefer, download the documents, Using online forms for your Belle Haven Mini-Grant Application and the itemized Budget Proposal form. The Using Online Forms document contains links to the online forms and the upload link to Drive so you can upload your completed budget proposal, photo residency ID image(s) and "before" pictures of your home/yard/safety improvement project.

- Mini-grant application (use the attached form)
- Mini-grant demographic survey (for statistical use only; use the attached form)
- An itemized budget with items and their expected costs ( use the attached form) If using a licensed contractor, please provide a quote. If you are doing the work yourself or purchasing materials, please attach an estimate of materials and any additional documentation of expenses.
- Photo ID and/or Proof of residential status (copy of driver's license and/or utility bill, etc.) Take a photo on your phone or camera. To upload directly to Drive make sure the file is in a JPG, PNG, TIFF, GIF format.
- If you have selected "curb appeal" home/yard/safety improvement, "before" pictures of your project must be taken. BHCDF will hire a photographer to take the pictures after the applications are submitted if needed.
